LET’S GET STARTED

Once we receive the completed form we will fax an order to your child’s primary care physician to be signed.

After we receive your completed Referral Form:

• An order is sent to your child’s primary care doctor to be signed.
• Our Intake Department will verify your insurance and ensure that your policy is active and get a general quote of therapy benefits.
• We will contact you to answer any questions you may have concerning insurance and the cost of services, as well as get your availability for therapy services.
• Once we receive the signed order back from your child’s doctor, one of our therapists will contact you to schedule the initial evaluation.
  
  

Please note, it can take up to two weeks to get the doctor’s order signed.
