All we need to get started is a COMPLETE Referral Form and a Physician’s Order signed by your child’s primary physician.

We will contact your child’s primary physician and request a Physician’s Order to be signed by them. Once all this information is received, we can schedule your child’s evaluation. It’s that easy!
We accept most major insurance carriers including, but not limited to: Aetna, Blue Cross Blue Shield, CIGNA, Humana, Medicaid, Tricare, UHC and Wellpath
* We are an in-network provider with Blue Cross Blue Sheild.
We will contact your insurance carrier and check the benefits on your policy. These benefits will then be discussed with you. You will know your financial responsibility before services are started.
If your policy offers coverage for therapy services, we will bill them first and then anything not covered will be your responsibility. An invoice will be mailed out to you in a timely manner.
If your insurance company does not cover therapy, you will be responsible for the full payment of services.
Milestone Therapy provides pediatric Occupational, Physical, and Speech Therapy to children ages birth to 21 years of age.
Our therapists travel to our client’s homes, preschools, and daycare centers.
